SCAPE (Sympathetic Crashing Acute Pulmonary Edema)

  • Clinical Findings:

  • Rales/Crackles on exam

  • Hypertensive

  • Tachycardia

  • Tachypnea

  • Vasodilate arterial side

  • BiPAP

  • Nitroglycerin

  • Goal BP is patient’s norm (don't need to cap decrease at 20%)

  • 1 SL Nitro is 400mcg, if given every 5 minutes, its approximately 80mcg/min 

  • Lasix

  • Kidney are typically poorly perfused

  • Can wait until BiPAP and Nitro control HTN
